This training will provide information on the EMbracing Progress & Opportunities, We Rise (EMPOWR) Program.The EMPOWR program aims to empower transition aged youth (TAY) with foster care experience by collectively identifying and amplifying strengths, coping skills, stabilizing housing and employment, increasing community support and embracing resilience in multiple life aspects. This program consists of a holistic treatment approach that is participant-led and delivered from a multidisciplinary team. The whole person is addressed by acknowledging the systems in which they function and the interconnected aspects that contribute to their wellness, including mind, body, spirit, identity and community connection. The team provides community-based therapy, case management and care coordination, and peer and family support through a team-based model. The multiple disciplines and identities brought together in the team also bring multiple perspectives and dynamism, so the team’s actions are flexible and respond to ever-changing circumstances or shifting goals.
