Participants will be able to:
  1. Identify the markers for a successful transition to adulthood by young adults in our society and in extended care.
  2. Describe the challenges in the implementing AB 12/212 extended care including finding placements, engaging young adults, and obtaining needed services.
  3. Describe the primary transition service needs for young adults in extended care including housing, education, permanency support, employment, and mental health.
  4. Identify case management and counseling practices for working with young adults in extended care.
