Foster Care Training

This training will discuss the various team-based care models and the services they provide to transition age youth. Team-based care programs are centered on participant goals, preferences and needs in which a team of multiple providers coordinate their services toward shared goals. This training will explore the needs of former foster struggling with unstable housing and limited access to supportive services. The training will review the services that participants receive from the team such as therapy, skill development, case management and care coordination. The training will also cover the importance of involving natural supports into case planning as well as the benefits from holistic practices.
